H Mole - Hydatidiform

Hydatidiform Mole also known as gestational trophoblastic disease occurs as result of abnormal degeneration of the trophoblastic cells (trophoblasts are the first cells that differentiate from a fertilized eggs). The differentiating cells do not develop normally and instead, they become filled up with a clear fluid and appears like small sacs (grape like vesicles). When this happens, the embryo does not proliferate anymore and growth is halted at this stage.
There are two different types of Hydatidiform Mole;
- Complete Mole
- Partial Mole
In complete mole, all trophoblastic swell and become cystic while in partial mole, some of the cells are formed normally but one part of the villi layers becomes swollen and misshaped.
Risks Factors Associated with H Mole
- More common in women who have low intake of protein.
- Women who are older than 35 years.
- More common in Asian women.
Hydatidiform Mole Symptoms
- The uterus becomes larger within a short time than it normally does. This is due to the rapid growth of the cells.
- Absence of fetal heart sound.
- When urine is tested for HCG (hormones presence during pregnancy), the urine turns positive but the HCG hormone level are usually higher compared to the amount found in normal pregnancy. The results continue to be positive until the time when the HCG would be expected to be decreasing.
- If the problem was not identified early in the course, by the second semester, bleeding may start (h mole is associated with second trimesters bleeding). The blood is usually dark brown or sometimes it may be fresh blood which is accompanied by grape like vesicles.
Treatment for Hydatidiform Mole
- Treatment of H mole includes use of suction curettage to remove the mole.
- Monitoring of HCG hormones which is done every 2 weeks until when the level returns back to normal.
- The testing continues after every 4 weeks for a period of 6 to 12 months. Increasing level of HCG witnessed in three times in row may suggest malignant transformation.
- Women are also advised to refrain from getting pregnant until after 1 year. This is to make sure that a positive pregnancy test is not confused with malignancy.
In rare occasion and whenever necessary, the physician may prescribe a prophylactic course of methothrexate; choriocarcnoma drug of choice (to prevent tumor from developing to cancer).
Women who experience bleeding are usually advised to bring a sample of the blood to the hospital, this is importance since the presence of grape like vesicles help in making the right diagnosis.
